Details:This .6 FTE commits to 24 hours per week Monday - Friday
primarily 8:00 am - 4:30 pm. Back up On call rotation of approximately
1-2 nights per month and holiday rotation. This position is eligible for
a \$10,000 recruitment bonus. This is not a temporary or seasonal
position, \$10K sign-on bonus for Weekend RN and Case Manager RN\'s,
tied to a 2-year commitment + Productivity Bonuses for all Case Managers

community resources, financial assistance and supportive care and
counseling to patients and families. What You Will Do: The Visit Nurse
works collaboratively with patients, nurses, social workers, physicians,
other practitioners, caregivers and community agencies in coordinating
and overseeing the interdisciplinary plan of care for a patient in a
home care/home hospice environment. Ideal candidate possesses excellent
interpersonal communication and negotiation skills along with strong
analytical and computer skills. Prior home health is beneficial. What
you will need: Associate\'s Degree in Nursing required. Bachelor\'s
Degree in Nursing preferred. Current State of WI or eNLC compact state
RN license. Valid State of WI driver\'s license with an acceptable
driving record. Current BLS (CPR) certification or ability to obtain
upon hire. Clinical experience in home health preferred. EPIC
