PT, Personal Care Aide II, Jamestown, NY Under supervision of a Registered Nurse, provides direct individualized support to assigned TBI Waiver enrolled individuals in an ethical, holistic and professional (non judgmental) manner. Provides support essential for individuals health and welfare. These services include cueing, prompting and supervision with activities of daily living and independent activities of daily living. $17.55/hour Subject to drug screening, medical exam, reference and criminal background checks. 2nd and 3rd shifts offer more hours. Must be available to work weekends.
