Schedule: PRN, require at least on average a shift a week with two shifts per month being a weekend day shift (Saturday or Sunday).

Shift : Days. Includes some weekends.

The role you’ll contribute:

Provides patient care in their home, following physician’s orders.

The value you’ll bring to the team:

· Initial assessment of patient in their home.

· Assures physician orders for care meet regulatory requirements.

· Ongoing evaluation of client in a holistic manner.

· Plans patient care in consultation with Case Coordinator Nurse.

· Supervision of LPNs and Home Health Aides and updates HHA Care Plans.

· Provide accurate, current documentation.

· Schedules daily visits to facilitate patient needs.

The expertise and experiences you’ll need to succeed:

Minimum qualifications :

· One (1) year experience in medical surgical setting.

· Current nursing license in the State of Florida.

· Certified in Basic Life Support/Adult.

Preferred qualifications:

· Home Care experience preferred.
