a part-time Business Health Resource Navigator in Arcadia, WI. This is
your opportunity to create a program facilitating Pilgrim\'s Pride
employees\' access to essential healthcare. What\'s Available? Part
time, 10-15 hours/week to start - there is a potential to expand hours
as this program grows Schedule: hours can vary based on business needs
Location: Onsite at Pilgrim\'s Pride in Arcadia, WI, with ability to
have some hybrid availability after training What Will You Do? Work with
Pilgrim\'s Pride employees to provide education, direction, and support
regarding how to access healthcare services at Gundersen Health System.
Build and maintain strong relationships with key stakeholders, HR
professionals and community partners. Work with Internal and External
partners to advocate for patient needs and make recommendations to
enhance the coordination of care. Develop, maintain, use, and share a
comprehensive, reliable, and accurate community resource database.
Provide outreach and education on access to healthcare to patients and
families to improve health and population health outcomes. What Do You
Need? 2 years of recent experience in a healthcare or human
service-related field Experience with resource navigation or case
management experience is a plus Excellent communication and
organizational skills Valid driver\'s license and reliable
transportation Bilingual, Spanish & English speaking, preferred but not
