Job Description Summary | Provide individual, family and group psychotherapy and counseling. Conduct intake evaluations, develop and implement treatment plans. Consult with and make referrals to clinical psychiatrist as clinically indicated. Act as patient advocate in securing services related to patients’ social, environmental and psychological needs utilizing community agencies. Provide crisis intervention services for psychological problems as needed during the regular work schedule. Participate in the on-call rotation schedule within the CAPS Department. Provide consultation to the Syracuse Community Health providers on mental health aspects of patient care. Participate in clinical supervision, case conferences and departmental/unit meetings as scheduled. Maintain accurate, timely and up-to-date case notes for the medical records. Maintain and compile department records, files, logs and reports in accordance with Center policy. |

Minimum Qualifications | Graduate of an accredited school with a master’s or higher degree in Social Work. Current New York State license required. New M.S.W. graduates who are L.M.S.W. eligible will be considered after obtaining a Limited Permit. Must obtain L.M.S.W. within 12 months of receipt of limited permit. |

Physical Demands | While performing the duties of this job, the employee is occasionally required to stand; walk, sit; use hands to finger, handle objects, controls; reach with hands and arms; climb stairs; balance; stoop, kneel, talk or hear. The employee must occasionally lift up to 15 pounds. Specific vision abilities required by the job include close vision, distance and color vision, peripheral vision, depth perception and the ability to adjust focus. Requires corrected vision and hearing within normal range. |
