Care Manager Level 2 - Adult Health Home - <P> FLSA (Non-Exempt) <LI> QUALIFICATIONS: <P> Education/Experience <LI> Bachelor's degree in health or human service field <LI> Two years' experience working with special needs populations <P> Skills <LI> Good organizational and time management skills <LI> Good oral/written communication skills <LI> Good interpersonal skills <LI> Knowledge of community agencies and resources <LI> Ability to work with service providers <LI> Familiar with computer programs (e.g. Word, Excel) <LI> The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs. <LI> Ability to legally operate a vehicle in NY State <P> MAJOR FUNCTIONS: <LI> Conduct initial and on-going assessments <LI> Conduct care plan meetings with all relevant service providers <LI> Act as a linkage to community services including medical, behavioral, residential, benefit services etc. <LI> Promote health and wellness activities <LI> Must utilize software technology pertinent to program <LI> May need to participate in afterhours/weekend on call coverage <LI> Maintain required documentation <LI> Ensure that a person centered planning process is employed in delivery of services <LI> Link and connect to natural supports and community resources <LI> Provide crisis intervention as necessary <LI> Complete required reports/notes in a timely and professional manner <LI> Comply with all applicable regulations <LI> Ensure that individual's rights are protected <LI> Advocate for individuals <LI> Maintain documentation necessary to substantiate billing/funding <LI> Participate in and successfully complete all required staff training and development activities <LI> Represent the agency and program in the community and with other service providers <LI> Regular and substantial contact with individuals receiving services <LI> Participate in team meetings <LI> Perform other job related activities as directed by supervisor <LI> Understand and follow all program policies and procedures <P> Apply - Send resume and cover letter to: Catholic Charities, 232 Main Street, Binghamton, NY 13905 or via e-mail at hr@ccbc.net. <P> In compliance with all applicable government laws, rules, regulations and standards and all Catholic Charities Policies and Procedures the duties and responsibilities of this position include but are not limited to the above.
