University Minnesota Physicians seeks a Certified Physician Assistant or Nurse Practitioner to join the University of Minnesota Wound Care Team. The wound care APP should be wound certified and has management and oversight for wound care patients. They collaborate and partner with our Vascular surgery, PMNR and Podiatry team for an interdisciplinary approach for treatment and management of wounds and swelling.
Practice Details:

The PA or NP is an experienced member of the health care team who provides comprehensive health care services to patients with the supervision/collaborationof licensed physicians

Wound care treatment experience/knowledge and wound certified

Excellent assessment and care coordination skills in both inpatient and outpatient settings

Assesses health status for wound care patient populations using advanced practice knowledge and skills

Develops and implements an individualized patientplan of care including coordination of care between visits

Contributes to effective and efficient practice management policies and practices that support patient access to medical care

Proven ability to function at highest level within scope of practice

Patient education and pro-active disease management experience

Demonstrates ability to provide care or service adjusting approaches to reflect developmental level of population served

This individual must show commitment to exemplary patient care

Fully integrated Electronic Medical Record (EPIC)

Openings:0.8 - 1.0 FTE
Schedule

Monday-Friday

Location would be split between the Maplewood Vascular Center (1-2 days per week), Wyoming (2-3 days per week) and Princeton (1-2 times per month).
Position Qualifications:

Wound certified

Graduate of an accredited school of nursing and formal Adult Practitioner training program or a graduate of an accredited Physician's Assistant program.

Current MN RN license and be approved or eligible by the Minnesota Board of Nursing for prescription writing privileges or a current PA license with prescriptive authority through the MN Board of Medical Practice.

Certified through ANCC/AANP or eligible for ANCC/AANP national exam or through the NCCPA and meet threshold criteria/qualificationsfor Credentialing and Privileges.

Certification, Wound, Ostomy, Continence Nurse (WOCN) or PA Wound Care Certification (or ability to obtain within 3 months of employment)

Excellent written and verbal communication skills

Exceptional collaboration abilities

Understands and places importance on patient care/service

Detail oriented and self-motivated

Education focused and a creative problem-solver
Preferred Qualifications:

Two years prior experience in Wound Care.
