Description

Implement vocational services developed by Office for People with Developmental Disabilities (OPWDD). Supports may include, but are not limited to, job coaching, job development, educational support services, and other employment training.
Essential Duties and Responsibilities

  • Complete vocational intakes, observations, and/or evaluations to assist individuals with developing a plan to return to work based on interests, abilities, strengths and needs.
  • Serve as a job coach, to assist individuals in gaining skills needed to be successful in a completive job.
  • Assist individuals to understand and overcome personal, social, or behavioral problems affecting their vocational situations.
  • Transport clients in agency or personal vehicle as required based on program need.
  • Assist vocational services support team with breaking down potential barriers to employment such as, transportation options, discovery related activities and hours of operation.
  • Create service plans with measurable goals and objectives based on the individuals interests, abilities, strengths and needs.
  • In collaboration with Vocational Services Community Liaison, assist with research of possible job openings and opportunities for individuals by contacting employment services, newspaper want ads, previous contacts, cold calling, and any other means of reaching potential employers.
  • Assist with educational goals, college applications, GED, and loan process, to assist individuals with returning to school.
  • Assist with facilitating workshops, on topics such as job clubs, computer labs, work compliance, problem solving, safety, and other trainings.
  • Assist with job analysis and job coaching, matching potential jobs/internships that meets the individuals needs, skills and logistics of getting to the job site.
  • Assist individuals with interview preparation, resume writing, job application and follow-up.
  • Maintain close contact with individuals during job training and job development, intervening when necessary to resolve problems and evaluate placement adequacy. Interact with employees at worksite as needed.
  • Case Conference with employees, family, service coordinators, case manager, and production and team/site supervisors as needed.
  • Make necessary referrals for individuals or family to community resources including state vocational rehabilitation services and benefits counselor.
  • Meet standards for all documentation and work accuracy including timely completion of progress notes, assessments, service plans and authorization requests for all stakeholders.
  • Maintain individual records in compliance with agency and confidentiality guidelines.
  • Represent Catholic Charities at committees as desired and approved by supervisor.
  • Participates in regular individual supervisions and team meetings.
  • Other duties as assigned.

Qualifications

Education: High School diploma or GED. Equivalent combination of education and experience will be considered. **

Credentials: N/A

Experience: 1 year of experience within a human service organization, preferably work focused on serving persons with a disability.
