Patient Care Pharmacist (Evening Shift)

Works with the multidisciplinary patient care team to provide optimal pharmaceutical care to patients including, verifying inpatient prescriber orders, dispensing ordered medications, compounding IV and all other sterile products, and timely preparation and delivery of all patient orders.

Schedule: (Full-time, Benefits Eligible)

- 2:00 pm to 12:30 am (10 hours shift)

- Every other weekend

- Holidays that fall on your schedule

Responsibilities




Serves as pharmacotherapy expert, recommends, and evaluates individualized drug therapy, makes recommendations in an evidence-based and patient-specific manner, identifies potential adverse drug reactions and incompatibilities affecting patient outcomes.
Dispenses inpatient medications pursuant to prescriber orders, including determining the appropriateness of medication orders, reviews all possible adverse drug events, and considers patient allergies.
Provides information to health care practitioners, caregivers, or patients on the appropriate administration of medications, dosing, and selection.
Documents direct patient care, clinical, operational, and financial outcomes, and activities appropriately in the medical record.
Provides appropriate handoff for follow up on anticipated pharmacotherapy needs, including medication changes. Collaborates to ensure continuity of care and consistency of practice.
Focuses on accurate maintenance of the 'prior to admission' medication list and ensures accurate and timely communication regarding discharge medication-related needs.
Provides in-service education and training to staff and precepts or supervises those awaiting licensure.


Qualifications




KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED

Education Requirement:

Bachelor's Degree from an accredited college of pharmacy, required.
Doctor of Pharmacy, preferred.

Licensure Requirement:

Licensure, or licensure-eligibility, to practice pharmacy in the state of Ohio.

Certifications:

Board certification, highly preferred.

Experience:

A post graduate year 1 (PGY-1) or 3 years of hospital pharmacist practice experience (pediatrics preferred) with responsibility for direct clinical patient care like that provided through a PGY1 Residency.
Previous Nationwide Children's pharmacy interns require 2 years of hospital pharmacy practice experience.
