Responsibilities

Position Summary:


The Manager, Diabetic Program is involved in direct patient care and is accountable for the day-to-day coordination and leadership of the diabetes team. This position collaborates with clinic staff, Diabetes Medical Director, and leaders from Medical Staff to oversee the implementation and evaluation of programs and processes to provide patient care around diabetes management. Achieves the Diabetes Program goals and objectives by planning, directing, monitoring and evaluating the daily operation of all department functions and staff. Participates in the development and implementation of care and performance standards to ensure that identified patient care needs and regulatory requirements are met and/or exceeded. Serves as the regulatory liaison for program certifications. Provides leadership and participates in the development of evidenced based interdisciplinary practice and patient care models (PCMH) which includes the coordination of care across the continuum to ensure smooth transition for the patient including pre and post hospital care and follow up.


Responsibilities may include:
- Subject matter expert in the principles and techniques of Diabetes management. Assists in developing innovative diabetes management programs.
- Works with the Physician Champion to develop protocols for diabetes.
- Provides operational support for the implementation of post-acute care strategies to improve the outcomes of Mercy Medical Group's Diabetic population.
- Coordinates and implements strategies with clinic leadership with the primary goal of enhancing clinical management of the high risk diabetic patients.
- Uses data to establish metrics, monitor utilization trends required in an Primary Care Home Model , NCQA payer requirements for delegated medical groups, and various Diabetes Associations.
- Establishes and maintains ongoing collaboration with hospital partners, referring physicians and their staffs to ensure coordination of patient services.
- Oversees and directs the human resources cost, development and growth functions for Diabetes Program. This includes reviewing and approving personnel actions including recruitment, selection or promotion of staff, evaluations, and disciplinary actions.
- Reviews, prepares, analyzes, and presents reports and recommendations to management regarding operations and/or other applicable areas of interest in order to provide concise and accurate information that aids in decision-making. Develops, reviews, and monitors clinical, service and financial outcomes using performance metrics.
- Performs additional duties as assigned.


Qualifications

Minimum Qualifications:


- 3 years leadership experience in a comparable role in the health care industry (acute care, post-acute, or community health based care services). Demonstrated superior leadership skills in developing and cultivating a high performing team.
- Bachelors Degree in Nursing field (BSN).
- Clear and current CA Registered Nurse (RN) license.
- Strong written, verbal, and interpersonal commuication skills; Understands the health care continuum and case management; Strong customer service and organizational skils; Strong sense of urgency and ability to work effectively in a fast paced environment handling multiple priorities; Proficient in Microsoft Office Suite of products (Project, Access, Excel, PPT) and report writers.
