Purpose of Job Position

The primary purpose of this job position is to provide quality direct nursing care to the residents, ensure quality direct nursing care is being delivered by State-Tested Nursing Assistant, and perform duties as may be directed by your supervisor according to established nursing care policies and procedures and current federal, state and local regulations and guidelines that govern St. Luke and as may be required by the Director of Nursing.

Major Duties and Responsibilities

Personnel Functions

  1. Perform all assigned tasks according to St. Luke’s established policies and procedures, and as instructed by your supervisor.

 

  1. Ensure that activities and delivery of services help to support the mission of St. Luke Lutheran Community.

 

  1. Direct, monitor and supervise the day-to-day nursing activities of State-Tested Nursing Assistants on assigned unit to include daily rounds of your assigned unit; ensure STNA’s are performing their work assignments in accordance with acceptable nursing standards and St. Luke policy and procedure.

 

  1. Ensure compliance with departmental and facility policy and procedures by assigned nursing personnel.

 

  1. Attend and participate in scheduled training and educational classes, inservices and orientation programs.

 

  1. Attend departmental meetings, care conferences, unit meetings, etc. when required.

 

  1. Participate in surveys (inspections) made by authorized government agencies as necessary.

 

  1. Perform duties such as completing medical forms, reports, studies, charting, etc., as necessary.

 

  1. Cooperate with residents, families, and other resident services and departments when coordinating nursing services to ensure that the resident's total regimen of care is maintained.

 

  1. Maintain a cooperative working relationship with other department personnel.

 

  1. Place orders for equipment and supplies as necessary and as authorized by the Unit Manager.

 

  1. Give/receive the nursing report upon reporting in and ending shift duty hours.

 

  1. Complete accident/incident reports, as necessary, on staff, residents, and operational faults.

 

  1. Ensure adherence to the quality assurance program for the nursing department.

 

  1. Admit, transfer, and discharge residents as required.

 

  1. Attend and participate in continuing educational programs designed to keep you abreast of changes in your profession as well as to maintain current licensure requirements.

Charting and Documentation

  1. Perform routine charting duties according to established charting and documentation policies and procedures. Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident's response to the care.

 

  1. Document on Medication Administration Record (MAR) according to policy and procedures and audit MARS on a routine basis to keep free of significant errors.

 

  1. Transcribe physician's orders to resident charts, medications records, treatment/care plans, etc., and notify other disciplines as required.

 

  1. Report all discrepancies noted concerning physician's orders, diet change, charting error, etc., to the Director of Nursing or designee.

 

  1. Record new/changed diet orders.

 

Drug Administration Functions

  1. Prepare and administer medications as ordered by the physician.

 

  1. Verify the identity of the resident before administering the medication/treatment.

 

  1. Order prescribed medications, supplies, and equipment as necessary according to established policies.

 

  1. Notify the Director of Nursing, RN coordinator or designee of all drug and narcotic discrepancies noted on your shift.

 

  1. Ensure an adequate supply of floor stock medication, supplies, and equipment to meet the nursing needs of the residents.

 

  1. Ensure that narcotic records are accurate for your shift.

 

  1. Return drugs and narcotics as required according to established procedures.

 

Nursing Care Functions

  1. Implement and maintain established nursing objectives and standards.

 

  1. Administer professional and quality nursing care services and treatments on a planned basis.

 

  1. Assist in answering resident call lights in a timely manner and assist nurse assistants when necessary.

 

  1. Notify resident's responsible party, physician, and RN coordinator of changes in orders or resident's condition.

 

  1. Obtain sputum and urine specimens as ordered. Take and record TPR's, blood pressures, etc., as necessary.

 

  1. Ensure that established post-mortem procedures are followed. Assist family members as requested.

 

  1. Transcribe physician orders to resident charts, Kardex, medication cards, treatment/care plans, etc., and notify other disciplines as required.

 

  1. Report to the Unit Manager/RN Coordinator the status of residents, problem areas, and other related matters. Assist in the development and implementation of any corrective action.

 

  1. Meet with residents, and/or family members as necessary. Report any problems or deficiencies to the Unit Manager/RN Coordinator.

 

  1. Participate in the initial assessment upon resident admission.

 

  1. Participate in Interdisciplinary Team process including the completion of documentation and assessment for the MDS reporting system.

 

  1. Ensure timely completion of weight audits and report the results to the family, physician and dietary services.

 

  1. Ensure compliance with and provide documentation for Restraint Reduction programs.

 

  1. Encourage attending physicians to record and sign progress notes, orders, etc. Make rounds with physicians as necessary.

Safety and Infection Control

  1. Practice established infection control methods, safety precautions and universal precautions practices when performing all duties and using equipment and supplies.

 

  1. Report all hazardous conditions or equipment, and safety violations to the appropriate personnel immediately.

 

  1. Report all accidents/incidents to your supervisor on the shift in which they occur.

 

  1. Clean, disinfect, and return all resident care equipment to its designated storage area after each use.

 

  1. Report any communicable or infectious disease that you contact to the Director of Nursing and/or to the Infection Control Coordinator.

 

  1. Follow established isolation precautions and procedures, and policies concerning exposure to blood/body fluids.

 

  1. Ensure that your assigned personnel are following established infection control methods, safety precautions and universal precautions practice when performing all duties and using equipment and supplies.

 

Resident Rights

  1. Maintain the confidentiality of ALL resident information.

 

  1. Ensure that you treat all residents fairly, and with kindness, dignity, and respect.

 

  1. Knock before entering the resident's room.

 

  1. Review complaints and grievances made by the resident and make a written/oral report the Unit Director/RN Coordinator indicating what actions were taken to resolve.

 

  1. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the unit and shift.

 

  1. Ensure that all nursing care is provided in privacy.

 

  1. Immediately report any witnessed or suspected verbal, physical, mental abuse by an employee, visitor, family member, or resident toward a resident.

 

  1. Immediately report any witnessed or suspected abuse, misuse or misappropriation of resident funds, belongings, etc., to the appropriate personnel.

 

Working Conditions

  1. Sits, stands, bends, lifts, and moves intermittently during working hours.

 

  1. Subject to frequent interruptions.

 

  1. Involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.

 

  1. May work beyond normal working hours, on weekends and holidays, and in other positions temporarily, when necessary.

 

  1. May be exposed to infectious waste, diseases, conditions, etc., including the AIDS and Hepatitis B viruses.

 

  1. Communicates with nursing personnel and other department personnel.

 

Education/Experience Requirements

  1. Must possess a high school diploma or equivalent.
  2. Must be, as a minimum, a graduate of an approved LPN or RN program.
  3. Previous long-term care experience preferred but not required.

 

Specific Requirements

  1. Must possess a current, unencumbered license to practice as a RN or LPN in this state.

 

  1. Must be able to read, write, speak, and understand the English language.

 

  1. Must possess the ability to make independent decisions when circumstances warrant such action, to follow instructions, and to accept constructive criticism.

 

  1. Must possess patience and the ability to work effectively with personnel, residents, family members, visitors, government agencies/personnel, and the public.

 

  1. Must be able to relate information concerning a resident's condition.

 

  1. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.

 

  1. Must possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel.

 

Benefits: Paid-Time-Off

 

Wage: $23.55 - $26.50/ hour (+$1.00 Evening/ Midnight Differential)  Sign on Bonus/ Quarterly Perfect Attendance Bonus and Employee Referral Bonus Available
