Community Service Provider- Children Level 2 - <P> QUALIFICATIONS: <P> Education/Experience <LI> Master's degree in health or human service field preferred <LI> OR <LI> Bachelor's degree in health or human service field required and <LI> Two years' experience working with special needs populations <P> Skills <LI> Good organizational and time management skills <LI> Good oral/written communication skills <LI> Good interpersonal skills <LI> Knowledge of community agencies and resources <LI> Ability to work with service providers <LI> Familiar with computer programs (e.g. Word, Excel) <LI> The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs. <LI> Ability to legally operate a vehicle in NY State <P> MAJOR FUNCTIONS: <LI> Provide direct service to children and families in their home, at school or in the community <LI> Provide direct services to children and families one-to-one and/or in groups. Delivery of services will follow a plan of care developed by the client and family <LI> Ensure a person centered planning process in delivery of services <LI> Provide appropriate care and supervision of all individuals while on outings <LI> Provide crisis intervention as necessary <LI> Link and connect to natural supports and community resources <LI> Maintain required documentation and complete in a timely manner to substantiate Medicaid billing <LI> Attend supervision and staff meetings as necessary <LI> Participate in multidisciplinary team meetings for quality care of individuals and represent the agency in a professional manner <LI> Comply with all applicable regulations <LI> Advocate for and ensure the individual's rights are protected <LI> Provide face to face service to children at least 50% of working hours <LI> Participate in and successfully complete all required staff training and development activities <LI> Perform other job related activities as directed by supervisor <LI> Understand and follow all Agency and Program policies and procedures <P> In compliance with all applicable government laws, rules, regulations and standards and all Catholic Charities Policies and Procedures the duties and responsibilities of this position include but are not limited to the above. <P> Catholic Charities of Broome County is an equal opportunity employer.
