Community Connections Inc. is looking for a Job Coach to join our Employment Services team based in South Yarmouth, MA. In this role you will support individuals with disabilities by providing comprehensive career planning, interview assistance & on-the-job advice. You will partner with individuals & businesses throughout the Cape Cod Community & aid Community Connections in their mission to provide optimal independence & enriching services to people with disabilities. We are looking for a Job Coach with a commitment to compassion, mentoring others & serving the community.

What You'll Do as a Job Coach:
Create & implement employment plans
Place individuals in community-based jobs that are consistent with their employment service goal
Maintain relationships with community businesses to secure employment opportunities for individuals we serve
Provide supervision & job support
Document individuals' progress toward employment plan objectives & keep up-to-date case records
Develop & exp& opportunities for community integration to address the employment, social, & recreational interests of the individuals we serve

What You'll Need as a Job Coach:
High School Diploma or General Education Degree (GED)
Bachelor's Degree in human service or business field preferred
1+ years' experience &/or training working with individuals with disabilities
Valid driver's license, access to a motor vehicle, & satisfactory driving record
Ability to work a flexible schedule
Excellent written & verbal communication & interpersonal skills
Demonstrated ability to work independently
Strong computer & word processing skills
