Scheduled hours per pay: 80
- Performs entire Annual Wellness Visit (AWV) appointment directly with patients:
- Maintains AWV criteria and obtains updates from Center for Medicare and Medicaid Services (CMS).
- Provides reimbursable RN services.
- Conducts in-depth verbal interviews with elderly patients to obtain health sensitive information.
- Collects patient history.
- Graduate of an accredited school of nursing.
- Current State of Michigan Registered Nurse license.
