JOB REQUIREMENTS: JOB PURPOSE AND REPORTING STRUCTURE: Under the
direction of the CSP Director, the CSP/MA Case Manager oversees the case
management of chronically mentally ill clients (as diagnosed using DSM
criteria) within a program of assertive community treatment, providing
counseling and assisting to maintain their independent living in the
community, in accordance with company policies and procedures. ESSENTIAL
DUTIES AND RESPONSIBILITIES include the following. Other duties may be
assigned. Participation in daily case consultation meetings within a
multidisciplinary treatment team. Conduct on-going assessment meetings
with clients in office or in the community. Ability to differentiate DSM
diagnoses. Provide supportive psychotherapy to assigned caseload.
Provide individual psychotherapy as needed using various treatment
modalities such as Dialectical Behavioral Therapy, Cognitive Behavioral
Therapy, Solution Focused Therapy, Harm Reduction, and Procovery /
Recovery models. Ability to assess and intervene in crisis situations
that may result in client harm to self or of harm to others in the
community. Knowledge regarding the use of psychopharmacology for the
treatment of a variety of client conditions. Assist clients with medical
requirements, such as their medications, doctor appointments, etc.
Observe client set up with medication, and assess their psychological
status and stability. Ability to assess a variety of AODA problems such
as abuse and /or dependency issues and to effectively attend to
client\'s needs using a variety of treatment modalities such as
Cognitive Behavioral Therapy, Harm Reduction, and Motivational
Interviewing. Assist clients with money management, bill paying,
budgeting, etc. (CSP provides Representative Payee services for most
clients). Assist with filling out other paperwork, such as leases,
social security, etc. Transport clients to and from appointments. Make
telephone calls on the clients\' behalf. Assist clients with grocery
shopping, laundry, cooking, and other daily living requirements that are
needed to ensure that client\'s are able to reside in the community in
the least restrictive setting possible. Complete assigned sections of
the In-Depth Assessment within designated timelines. Develop and
complete treatment plans and goals for clients. Maintain all required
case notes and records. Coordinate with other community
resources/services to assist the clients. Arrange social/recreational
activities for the clients. Assist clients with finding housing when
