The organization is seeking an LPN to work as a Medical Coordinator for our residential program which consists of 4 IRAs. Candidates should have knowledge and experience with OPWDD regulations and procedures, possess assessment/evaluation skills, and have experience working with individuals with intellectual and developmental disabilities. Applicants should have excellent communication and organizational skills and be proficient with multi-tasking and working together as a team member. 


Responsibilities include:
 Prepare clinical consultation forms for all medical appointments.

· Maintain scheduling of medical appointments/medication records/and ensure RN/MD treatment orders are implemented for all individuals. 

· In collaboration with the RN and Residence Manager, ensure that sufficient staff are available for each appointment so it can be scheduled.

· Review all consults immediately upon return to the facility per the agency’s guidelines. 

· Ensure that all medical appointments are up to date

· Develop weekly/monthly (as determined by the RN and Residence Manager) medical appointment calendar

· Maintain and obtain AMAP certification

· Ensure that all prescriptions are submitted to Grassy Sprain Long-term Pharmacy upon receipt request. Count and record medications upon delivery; if an AMAP receives the medication, ensure it has been counted and recorded correctly.

· Process medication/treatment order(s) in house and with the pharmacy. Received delivery

· Check the Medication Cart/ Room weekly to ensure sufficient medications for a minimum of ten days.

· Reorder medication when the supply is insufficient for ten days.

· Review that Long Term Care Pharmacy receives all original prescriptions.

· Daily MAR Checks.

· Review and log all control substances: review the log for control substances minimally weekly to ensure compliance and validate count.

· Ensure the Medication Room/ Cart is clean and orderly and that all supplies are sufficient; * Order medical supplies as needed*.

· Ensure that all AMAP signatures are on the MAR before the first administration each month: file Mar documentation monthly.

· Preparation of off-site medication (home visits, trips, etc) and inclusion of documentation of the same for the individual by OPWDD regulations and agency policy and procedures. 

· Assist RN in the maintenance of individuals' medical binders

· Assist RN in completion of forms; infection control (monthly); medical appointments (monthly); Nursing report; resident report (monthly)

· Ensure pharmacy review is scheduled and completed quarterly.

· Review/ monitor Bowel Movement charts daily; report any issues to the RN and Manager.

· Appointments/activities and review all outstanding referrals/reports/orders 

· Attend Clinical Team meetings/ISP meetings/Annual and Semi-Annual meetings. 

· Organize consent for the treatment process 

· Additional duties as required

Qualifications:
Graduate of accredited Licensed Practical Nurse (LPN) program, Current NYS Nursing License

Strong Organizational and interpersonal skills

Good oral and written communication skills.
