Job Description

Job Title: Care Manager (RN)

Job Location: Member Facing - Dayton, OH

Job Duration: 8 Months (Potential to extend)

Responsibilities:

Required to do field visits (member-facing)

Two different types of caseloads (one working with the over 60 waiver population; the other will be working with the under 60 population)

Assessments face-to-face visits for maintaining safety welfare; audit their caseloads

Must haves: Behavioral Health and experience with working with mental health and substance abuse

Education/Certification

Required: Associate's degree of Nursing

Preferred: N/A

Licensure

Required: RN (OH or compact)
