Description and Requirements

Primary Responsibilities

  • Assess community health needs and recommend / prioritize educational programs
  • Support in the development and execution of health educational programs
•Support the creation of objectives and goals
•Work with internal team to develop relevant content / materials and facilitate work shops, training sessions and community events
  • Work with team lead and project associate to track identified metrics and monitor progress
  • Program effectiveness
  • Work with clinical and care management team to develop culturally sensitive and competent educational materials and resources
  • Work to promote healthy behaviors in the community (based on area of focus)

Essential Skills

  • High school diploma or GED Equivalent
  • Certificate in Health education (Certified Health Education Specialists) or BA Public Health, Health Education or Related Field
  • Bi-lingual - Spanish
  • Should be a community health worker
  • Experience working with diverse communities, including faith-based organizations and Local healthcare providers
  • Strong communication, interpersonal and presentation skills
  • Knowledge of health education principles, practices and cultural competency
  • Program planning and evaluation
