well as each other. Position Summary: This position is responsible for
helping our individuals understand, navigate, and grow through the CCS
program. This Care Coordinator meets and works with the individuals,
service providers, and other team supports that assist in the
individual\'s progression through the CCS program. The Care Coordinator
receives consultation from mental health and substance abuse
professionals while working on the individuals\' individualized recovery
plan. Essential Functions: Establish individual\'s eligibility by using
a Mental Health/AODA Functional Screen. Assist the individual in
identifying services from the CCS service array appropriate to address
recovery needs. Broker services as needed with other providers in the
CCS network. Work with the individual to identify and build a recovery
support team of professionals as well as natural supports. Work with the
individual to make an individualized recovery treatment plan. Facilitate
recovery meetings with the individual and the team. Work with the
individual to prioritize and obtain the goals set forth in the
individual recovery plan. Follow-up with service providers to ensure
