Job Summary
Under minimum supervision, performs one-on-one patient centered needs
assessment, care coordination, and behavioral health education to patients
and other parties that may be involved in a patient's care, such as
friends/family members and other professionals. The Behavioral Health
Educator will conduct telephonic screening of patients for substance use
disorders, depression, anxiety, and overall psychosocial functioning,
and provide referrals and guidance on accessing appropriate internal and
community resources. Other duties include collaborating with hospital wide
health education initiatives as needed, identifying new and existing
resources and organizations that serve patient health needs, and maintaining
