ABOUT THE JOB
The Care Coordinator is responsible for serving as the central facilitator to patients assigned to their care. The general function is to guide and coordinate care and collaborate with primary care providers, RNs, and clinic staff in identifying needs, organizing activities, and organizing plans of care for patients with chronic conditions. The Care Coordinator is an advocate for the patient and works to link them with health and community resources that provide a range of services, promote self-management, improve health, and reduce disparity. This position also assists patients to achieve health goals and improve health care outcomes.
Responsibilities:
Assesses the health and psychosocial needs of patient/family.
Uses interviewing and observation skills to determine common variables and unusual problems affecting care.
Assist providers and staff with clinic flow, providing direct patient care when circumstances make that necessary.
Collaborate with providers, clinic leadership and support staff as well as other departments and agencies as appropriate to facilitate quality patient care.
Apply motivational interviewing skills, care coordination, and coaching principles to the care of patients with chronic/complex conditions.
Develop a patient centered care plan with patient, family members, and providers.
Facilitate care transitions.
Provide patient and family education.
Make necessary appointments for patients, identify barriers to attendance and mitigate them.
Connect patients with appropriate community resources.
Assist with development of clinical workflows that promote meeting quality measures, ACO and other program standards.
Conducts telephone triage for patients calling in for same day appointments.
Work patient registries.
All other duties as assigned.
ABOUT YOU
Graduate of an accredited school of nursing with an associate's degree in professional nursing (preferred)
Current RN licensure within the State of Minnesota (preferred)
CPR Certification (required)
Experience with health care technology clinical applications (preferred)
Knowledge of health care practice (required)
Ability to work with a diverse patient population (required)
Demonstrated problem solver and self-starter (required)
Ability to work independently without direct supervision (required)
