TITLE: Direct Care Provider

TYPE: Part Time/Full Time- No Benefits

LOCATION: Spencer- Private home

GENERAL JOB DESCRIPTION:

Assist 2 individuals with limitations in their home to meet their daily needs in all areas of their daily life. This can include but is not limited to independent living skills, medication management, household tasks, transportation/ community integration and supervision/social interaction.

DUTIES:

• Provide guidance and support to complete all steps to meet their needs to ensure proper diet, hygiene and health needs

• Remind individual to take medication and assist with set up when needed

• Monitor and report health concerns

• Supervise and assist with all household tasks which vary each day and how much the individual can do at that time. (Tasks will be completed by staff if necessary.)

• Transport individuals to appointments and assist with communication so providers understand individuals needs

• Transport and supervise individual in community activities

• Assist and ensure individuals safety at home and in the community

• Encourage individual to do as much as they can independently but ensure tasks are completed with assistance as needed

• Supervise and assist with pet care as needed

QUALIFICATIONS:

• Have a valid drivers license

• Have a car with insurance coverage

• Be able to follow written and verbal directions

• Be able to complete computer documentation in a timely manner after service delivery

• Be caring and compassionate

• Have an understanding of the individuals needs and abilities

• Keep confidentiality at all times

• Be open to suggestions and have a willingness to learn
