commits to 40 hours per week Monday - Friday primarily 8:00 am - 4:30
pm. Back up On call rotation of approximately 1-2 nights per month and
holiday rotation. This position is eligible for a \$10,000 recruitment
bonus. This is not a temporary or seasonal position, Starting from
\$62,483 annualy- increases with expereince! \$10Ksign-on bonus for
Weekend RN and Case Manager RN\'s, tied to a 2-year commitment +

social workers offer assistance with community resources, financial
assistance and supportive care and counseling to patients and families.
What You Will Do: The RN Case Manager works collaboratively with
patients,social workers, physicians, other practitioners, caregivers and
community agencies in coordinating and overseeing the interdisciplinary
plan of care for a patient in a home care/home hospice environment.
Assesses, plans, facilitates and evaluates timely coordination of high
quality, cost-effective care. Responsible for improving the health of
patients dwelling in their own homes. This requires a nurse who is
comfortable working independently, enjoys teaching, is a critical
thinker and has strong communication skills. The RN Case Manager
provides hands on care in the home environment and offers autonomy,
flexibility and the luxury of caring for one patient at a time.
