Job Summary: We are seeking a compassionate and skilled Home Health Nurse to provide personalized 1:1 care to patients in the comfort of their homes. The Home Health Nurse will work closely with individuals of varying ages and medical needs, delivering high-quality medical care, emotional support, and education to both patients and their families. This role requires a strong sense of empathy, excellent clinical skills, and the ability to adapt to different environments and situations.
Responsibilities:
- Conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the healthcare team.
- Administer medications, treatments, and therapies according to prescribed protocols and physician instructions.
- Monitor patients' vital signs, symptoms, and overall condition, and promptly report any changes or concerns to the appropriate healthcare providers.
- Provide wound care, catheter care, and other specialized nursing interventions as required.
- Offer emotional support, counseling, and education to patients and their families regarding their medical condition, treatment options, and self-care techniques.
- Assist patients with activities of daily living, including bathing, grooming, dressing, and mobility, while promoting independence and dignity.
- Coordinate with other healthcare professionals, such as physicians, therapists, and social workers, to ensure holistic care and continuity of services.
- Document all patient assessments, interventions, and outcomes accurately and timely in the electronic health record system.
- Collaborate with community resources and agencies to facilitate patients' access to additional support services and resources as needed.
- Maintain strict confidentiality and adhere to ethical standards in all interactions with patients and their families.
Qualifications:
- Registered Nurse (RN) license in good standing with the appropriate state licensing board.
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 1 years of clinical experience in nursing, with demonstrated proficiency in home health or related field.
- Current Basic Life Support (BLS) certification; additional certifications such as Advanced Cardiac Life Support (ACLS) may be required depending on the patient population served.
- Strong assessment, critical thinking, and problem-solving skills.
- Excellent communication and interpersonal skills, with the ability to establish rapport and build therapeutic relationships with patients and families.
- Ability to work independently and as part of a multidisciplinary team in a dynamic and fast-paced environment.
- Proficiency in using electronic health record (EHR) systems and other healthcare technology tools.
- Valid driver's license and reliable transportation for home visits.
- Flexibility to work evenings, weekends, and holidays as needed to accommodate patients' schedules and ensure continuity of care.
Physical Requirements:
- Ability to lift and maneuver patients safely, with assistance as needed.
- Prolonged periods of standing, walking, and bending during home visits.
- Visual and auditory acuity to assess patients' condition and respond to alarms or emergency situations.
- Ability to tolerate exposure to potentially infectious agents and hazardous materials in accordance with standard precautions and safety protocols.
