Queen’s North Hawaii Community Hospital is a 35-bed rural acute care hospital located in Waimea (Kamuela), Hawaii Island. More than 300 employees and over a dozen employed physicians staff the main hospital, as well the outpatient clinics located in the Lucy Henriques Medical Center, at our Primary Care Clinic located near Longs Drugs, and at our Specialty Clinic located in the Parker Ranch Center.

Responsibilities

 

JOB SUMMARY/RESPONSIBILITIES:
•  Implements, maintains and provides clinical pharmacist services including: clinical, educational, informational and distributional functions for newborn, infant, pediatric, adolescent, adult and geriatric patients of Queen’s North Hawaii Community Hospital (QNHCH). 

TYPICAL PHYSICAL DEMANDS:
•  Essential: Standing, sitting, stooping, bending, walking, finger dexterity, seeing, hearing, speaking. 
•  Frequent: pushing/pulling usual weight of up to 40 pounds, lifting or carrying up to 30 pounds, reaching above, at and below shoulder level, frequent gripping of an object. 
•  Continuous: repetitive arm/hand motions. 
•  Occasional: kneeling, crawling, climbing stairs, walking on uneven ground, squatting, static gripping of an object for prolonged periods, pushing/pulling usual weight of up to maximum 50 pounds. 
•  Operate pagers, computers, printers, typewriters, telephones, fax machines, dispensers, laminar flow hoods, and other pharmaceutical and office equipment.

TYPICAL WORKING CONDITIONS:
•  Not substantially subject to adverse environmental conditions. 
•  Subjected to noise/vibration from air conditioner, printers, time stamp machines, telephone, etc. 
•  Exposure to hazardous drugs, antibiotics, chemicals, dust, odors and cleaning/ disinfectant fumes.
•  Work environment is fast paced, congested and may be stressful due to workload. 
•  Interacts with patients, nurses, physicians and the general public. 
•  Works on multiple tasks at the same time. 
•  May be exposed to communicable diseases.

MINIMUM QUALIFICATIONS:
A.  EDUCATION/CERTIFICATION AND LICENSURE:
•  Current Hawaii State license as a Pharmacist.
•  Doctor of Pharmacy degree preferred.
•  Post-graduate clinical residency accredited by the American Society of Health-System Pharmacists preferred.

B.  EXPERIENCE:
•  Clinical hospital pharmacist experience preferred.  Preferably with experience in rounding with a medical team, monitoring of patient drug therapy, pharmacokinetic dosing and patient counseling and education.
•  Experience in unit dose and IV services preferred.
•  Ability to communicate professional expertise to all levels of the health care team.
