Merakey is hiring an LPN or RN to work with our Adult Mental Health program in Delaware County.
Earn up to $30.10 as an LPN or $42.96 as an RN, plus we are offering a $4,000 bonus.
Position Summary:
Within key functions in the Merakey Health Home model, the nurse navigator serves as a member of the Adult Targeted Case Management team.  In this role, the nurse navigator provides care management, care coordination and health advocacy for consumers to ensure that they receive the behavioral healthcare and physical healthcare they need.  The specialized case management services of the nurse navigator are provided to all individuals served by an Adult TCM team whether they participate in the on-site primary care services or receive those services from a PCP in the community.

Requirements

The ideal candidate will possess the following qualifications:
License as a registered nurse (RN) or licensed practical nurse (LPN) in Pennsylvania;
Medical experience working as an RN or LPN plus minimum one year experience in either inpatient or outpatient mental health setting;
Experience with addictive diseases and/or community mental health preferred.
Clean, valid driver's license and clean driving record.
