As a Case Manager Senior with our Patient Navigation and Business Services (PNBS), you will be part of a dedicated team of healthcare professionals committed to delivering world-class care to patients from all corners of the globe. In this role, you will have the opportunity to expand your knowledge of all specialties within the UCLA Health system as you collaborate with various medical experts, multi-disciplinary teams, and clinics to ensure a smooth transition of care for all incoming international patients. While helping to make a positive difference in the lives of our international patients, you will have the opportunity to enhance your understanding of an array of medical diseases and conditions treated at UCLA Health as well as advancements in medical technology and treatments.
Using your advanced practice nursing skills, you will be responsible for assessing and coordinating care for a diverse group of patients. The Case Manager Senior is responsible for processing incoming patient referrals, including reviewing medical records, completing clinical assessments, and arranging and participating in MD-to-MD case reviews with physicians abroad and UCLA physicians to conduct thorough case reviews. They will provide education to the patients and their families, and is responsible for facilitating hospital transfers for all internationals patients. As the Senior Case Manager, you will be a main point of contact for all incoming international patients and will be responsible for guiding patients throughout the continuum of care at UCLA Health.

Salary Range: $135,490 to $178,691 Annually
Qualifications:
- BSN degree and California State RN license required.
- Ability to complete a Nursing physical assessment.
- Skill in developing a plan of care including setting goals, monitoring and documenting progress.
- Ability to implement interventions that are pertinent to goals established in the plan of care.
- Ability to evaluate the effectiveness of interventions toward the attainment of established goals and to recognize the need to reassess or reset goal parameters.
- Knowledge base to provide education to patients, families, international referring physicians and other staff regarding clinical and patient care issues.
- Exceptional verbal and written communication clinical skills and expertise in triaging incoming inquiries and referrals with quick turnaround times.
- Ability to collect and relay information with accuracy and diplomacy, and effectively communicate verbally and in writing.
- Working knowledge of medical and scientific terminology to guide patients through the multiple specialties available at UCLA Health, and triage patients to the different teams and specialty services through review of medical records, discussions and conversations with the international patients and their treating/referring physicians in their home country, and to respond quickly to patient needs and physician directions.
- Ability to monitor patient compliance with plan of care and ensure care gaps are identified and resolved.
- Must possess cultural intelligence to work with diverse cultures and situations, and capability to cross boundaries with sensitivity and awareness.
- Ability to recognize a patient’s needs for referral to support services such as social work or psychology.
- Knowledge of UCLA resources for all patients, e.g. acute or outpatient rehab; SNF and rehab; home health services; hospice care; transportation services, DME etc. is strongly preferred.
