POSITION SUMMARY:
The Direct Support Professional / Driver is responsible for a variety of duties related to the overall care of individuals with developmental disabilities who attend Alpha Care Ltd. programs. This includes direct care of individuals and implementation of individual plans (IP) and behavior plans (BP). Direct Support Professionals are required to perform all duties in accordance with State of Ohio regulations and Alpha Care Ltd. policies and procedures.
DRIVER’S JOB REQUIREMENTS
· Valid driver’s license and clear driving record.
· Driver’s must immediately notify the agency in writing, should the driver accumulate six or more points on their driving record or if their driver's license is suspended or revoked.
· Ability to drive company vehicles.
Ability to lift/push/pull 50lbs.
Ability to work on holidays and during inclement weather.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Training
· Completes and maintains all required State mandated trainings such as CPR / First Aid, Alternative Thinking and all DODD required trainings such as Major Unusual Incident / Unusual Incident Training, OSHA, Health and Safety, Driver Training, etc..
· Attends meetings and training sessions as required by DDA and FSF;
· Completes annual trainings related to individuals’ ISP, and related update and changes to these plans;
Daily Programing
· Provides transportation to and from Alpha Care Ltd. Center;
· Assists in the implementation of individual service plans, services and supports, outcomes and goals.
· Assists individuals in the pursuit of vocational, recreational, social and community activities based on their preferences and individual plans or specific programming.
· Assists and encourages individuals in the establishment of social networks;
· Promotes participation and involvement in community outings;
· Works all assigned schedules, including but not limited to: arriving and departing on time, requesting time off in advance, and notifying your supervisor of changes to your assigned work schedule;
· Ensures each individuals’ rights are protected;
· Treats individuals with dignity and respect;
Daily Activities
· Communicates with HPC staff and Alpha Care Ltd. updates regarding assigned
individuals;
· Observes individuals for any changes in behavior or health and reports these as needed;
· Assists and encourages individuals to care for immediate personal needs such as toileting, washing their hands, and eating;
· Assists with meals based on individual needs and preferences per ISP;
· Keeps assigned areas & vehicles – both interior and exterior –clean, organized, and tidy and encourage individuals to help with upkeep as appropriate;
· Teaches individuals vocational skills, which are practical skills specific to a career, trade or job role (these can be soft skills and/or hard skills), as specified in their service plan;
Communication
· Observes individuals for evidence of injury or bruises and evaluates for changes in emotional and physical status;
· Reports any noted medical related and unusual incidents to the
Coordinator and Supervisors immediately and complete a written unusual incident report form;
· Acts as a representative for Alpha Care Ltd. in a professional manner when required, to co-workers, Individuals, Families, Coordinators, State officials, Community Partners, Local Businesses, and Employers;
· Maintains HIPAA and the confidentiality of each Individual;
· Communicates when items required for daily operations need replaced, restocked or checked;
Teamwork
· Assists the Operation Managers/All Coordinators in the preparation for the annual Individual Planning Meeting;
· Treats all individuals and coworkers and with dignity, respect, and fairness;
· Provides support to other team members when individuals are having behavioral
issues;
· Provides feedback after incidents occur and discuss options and ideas with team;
Documentation
· Documents all program data, including but not limited to: individual plan data, behavior data, community integration docs/logs, assessments, transporation documents, mileage logs, maintenance requests forms, and vehicle checks etc.
· Documents individuals’ health (i.e. seizure charts, weight, bowel movements, fluid intake.);
· Ensures that all daily documentation is completed accurately and in a timely manner;
· Completes MUI/UI documentation as required when incidents occur; documentation should be clear and concise.
· Clocks in and out before and after every shift and submits Leave Request forms when applicable and as specified in the Employee Handbook
Safety
· Assists individuals when entering and exiting vehicles at all times, assists individuals on community outings, as well as entering and exiting the building safely;
· Supervises meal times based on individual needs, especially those identified as a risk for choking;
· Observes appropriate staffing ratios and level of care as directed by the ISP;
· Reports unsafe conditions, environment, and equipment to the appropriate supervisor:
