looking for an Advanced Practice Provider (NP or PA) who takes an active
role in identifying and creating opportunities to educate, motivate and
collaborate with staff and students in their professional development.
PHC is looking for someone who renders advanced clinical practice
services to all ages under a collaborative agreement with a designated
PHMGphysician. Responsibilities include: assessment, diagnoses,
treatment and when necessary referral to others for further care.

their care. Conditions managed include both acute and chronic illnesses
with management of co morbidities in the ambulatory care setting.
Obtains medical histories and ensures relevant documentation in
accordance with PHMG standards, the state of Wisconsin, PHMG Scope of
Practice and collaborative agreement with PHMG physician. Interprets and
evaluates patient care data in order to determine management and
treatment of diagnoses in accordance with evidence based practice,
