JOB REQUIREMENTS: RN Team Care Coordinator 0830-1700 Monday, Wednesday

the scope ofa Registered Nurse.Delivers appropriate nursing care and
proceduresinclinic setting. Performs nursing actsdirected by a
physician, physician assistant, or nurse practitioner.Provides patient
triage, develops and monitors care management coordination processes,
patient teaching, and patient follow-up when appropriate.Serves the
needs of the patient panel of their Team.Acts as aresource for the
clinic LPN/MA\'s and other ancillary professionals.Act as the resource
for data gathering and assessment.Triage walk-in and call-in patients
and provide appropriate direction to the patient. Participates in the
teaching of residents and patients. Requirements: Associates in Nursing
