Job Title: Licensed Practical Nurse (LPN)
Shifts Available: Various shifts including 7am-3pm, 3pm-11pm, and 11pm-7am
Location: Huntington, NY - Nursing & Rehabilitation Center
Salary: Competitive, commensurate with experience
Health Source Group is looking for Licensed Practical Nurses (LPNs) to join our team at the Huntington Nursing & Rehabilitation Center. As an LPN with Health Source Group, you will play a crucial role in providing high-quality care to our residents, ensuring their well-being and comfort.
Responsibilities:
Provide nursing care under the direction of RNs and physicians.
Administer medications and treatments as prescribed.
Assist in developing and implementing care plans for residents.
Monitor and report changes in residents' conditions to healthcare providers.
Maintain accurate documentation of nursing assessments, interventions, and outcomes.
Collaborate with multidisciplinary teams to ensure comprehensive resident care.
Educate residents and families on healthcare procedures and treatments.
Adhere to facility policies, procedures, and regulatory standards.
Qualifications:
Current New York State LPN license.
Graduation from an accredited LPN program.
CPR certification.
Strong communication and interpersonal skills.
Ability to work effectively in a team environment.
Experience in long-term care or similar healthcare settings preferred.
