Job Description:The Behavioral Health Provider assesses patients and/or
patient's family, caregivers, and/or legal representatives and
arranges for needed interventions. Plans interventions to help patients cope
with social, emotional, economic, and environmental problems and
provides short term treatment planning for mental, emotional, and
behavioral disorders, conditions, and addictions.
Scope
The Behavioral Health Provider will work at Intermountain Health Amber Creek
Clinic, Fort Lupton Clinic and Quail Creek Clinic. This position provides
care appropriate to the established plan of care in the assigned patient
population.
Job Essentials
Assesses the patient's medical history, mental health history, and
current presentation to understand the patient's needs from treatment.
Organizes and prioritizes daily work by assessing new, current, and
discharging patient needs in area(s) of responsibility.
Diagnoses and treats various mental, emotional, and behavioral disorders.
Performs social work consultation for patients and caregivers. Conducts
psychosocial assessments. Provides supportive, crisis bereavement and other
social work interventions as necessary.
