Assists in departmental management, including, operations, patient care delivery, clinical service determination, and regulatory compliance, to achieve performance, medication safety, and quality control objectives.
Assist with the creation and management of evidenced-based, rational, cost-effective drug therapy protocols, standard operating procedures and pharmacy polices.
Provides comprehensive, patient centered pharmacotherapy for inpatients making patient specific drug therapy recommendations. Activities include but are not limited to: proactive review of patient profiles, participation in patient care rounds, contacting and conferring with providers to optimize medication therapy, completion of pharmacotherapeutic consults, participation in the medication reconciliation process at care transitions, and promotion of economically conscientious use of medications. Documents all pharmacy interventions and outcomes.
Provides pharmacy operation staffing support. Review, interprets and evaluates medication orders of accuracy and completeness. Identifies, prevents, and resolves potential or actual drug related problems. Provides supervision of technical staff to ensure quality and accuracy of all dispensed products.
Ability to manage patients in an outpatient anticoagulation clinic, including operation of Point of Care testing, evaluation of results, adjustment of the patient's medication regimen and patient education.
Acts as an information resource to patients, physicians, nurses and other healthcare professionals regarding medication therapy and drug related information and literature.
