Job Summary
Under minimal supervision, provides individualized telephonic and/or face
to face transitional case management services for individuals who require
intensive support and monitoring, including medical, behavioral,
and/or substance abuse services on a time-limited basis during transition
from critical events such as incarceration, homelessness, and/or drug
use. Assists and educates patients to seek appropriate care and services at
Denver Health for outpatient follow-up and enrolling in care. Works closely
and collaboratively with community-based organizations, clinical
