Position Summary:

Responsible for the coordination of behavioral healthcare when a patient request accesses to Inpatient Behavioral Health. Oversees, guides, completes and follows up on Pre-admission screens for those who seek admission for individuals, physicians’ offices directing a client to the Behavioral Health Unit for admission or assessment of those individuals that present to the Emergency Department in a Psychiatric crisis.

 
 

Essential Functions and Responsibilities:  

  1. Acts independently, consistently exercises discretion and judgment in performing work which is predominantly intellectual and varied in nature.
  2. Navigates patients, physician offices, and affiliates, nursing units and emergency room through the Inpatient Mental health care system including community, individual and family education.
  3. Coordinates, develops and evaluates intake screening process.
  4. Presents pre-admission screen to psychiatrist and receives orders for admission.
  5. Ensures that a handoff communication following the pre-admission process.
  6. Collaborates with the health care team to plan and evaluate treatment options.
  7. Prepares and presents periodic outcomes reports for assigned patient population.
  8. Develops relationships and outreach programs with inpatient nursing units, emergency room and primary care physicians.
  9. Assists with the compliance of The Joint Commission, and any other regulatory requirements, such as Community Mental Health agencies.
  10. Communicates with third party payers for precertification of admissions and continues stay days, providing required information to payers and documenting payer contacts and authorization details.
  11. Applies specified criteria to all patients requiring precertification to determine appropriateness of admission and level of care including observation status.  Performs continued stay reviews utilizing approved criteria as indicated.
  12. Assists physicians and case managers to identify alternatives to admission for cases that do not require acute care.

Daily Work Times: 7:30 a.m. – 4:00 p.m.

Scheduled Bi-weekly Hours: 80

Qualifications:
Required:
  • Must have current Michigan Registered Nurse License.
  • BLS issued by American Heart Association or Red Cross Certification.
  • Behavioral Health Certification within twelve (12) months.
  • One or more years clinical experience with inpatient Behavioral Health Patients.
  • Graduate of an accredited school of nursing.
 
 
Preferred: 
  • Bachelor Degree in Nursing.
  • Prior Experience as a Behavioral Health Navigator
  • Behavioral Health Certification within twelve (12) months
