Job Description

 
CASE MANAGER HOMELESS HOUSING Responsible for daily operations of the assigned scattered homeless housing. Makes referrals to collaborative agencies in support of client services. Responsible for on-site visits to clients, handling of identified issues and reporting on progress and status to Residential Manager. Essential Functions: Conduct intake, comprehensive needs assessment and develop individual residential needs plan addressing clients needs and goals Review residential care plans quarterly Oversee activities that support the development of independent living skills for all assigned participants. Provide and/or organize life skills training Conduct on-site visits at a minimum of 1 visit per month per client Coordinate services for individual clients to ensure clients safety and achievement of identified goals, including food purchases if needed Ensure that clients are receiving necessary services from appropriate agencies/community services Assist clients with applying for public and private benefits Complete clients progress notes and other documentation appropriately and in a timely manner Attend sponsor meetings as needed Input clients data in HMIS database Be on 24-hour call as needed for crisis intervention Attendance and Punctuality
